» Group Leader ManualThis document includes what to expect, payment timelines and information, forms required, and more. Please download or print this document and read it thoroughly to prepare your group to serve.

» Group Information Form (Excel – Preferred) / Group Information Form (PDF)The Group Information Form is filled out by the group leader and lists all of your participants attending the project. It is used by the project coordinator for room and squad assignments. This form must be sent to the PC before the project.

» Group Transportation Form – The Transportation Form is filled out by the group leader and lists the forms of transportation that the church is bringing to the project and which adults can serve as drivers of these vehicles. Please also include any personal vehicles driven by adults. This form must be sent to the PC before the project.

» Participant Form – The form should be completed and signed by every participant and parent/guardian. Participants under the age of 18 must have the form notarized by a notary public, before leaving for the project. The form gives Mission Serve important medical information and authorization for emergency medical treatment. Group leaders are responsible for bringing the Participant Forms to the project and presenting them to the summer staff at registration.

» Church Group Devotions – This document provides questions for the nightly youth group devotions that happen after worship at a project. You will receive another copy of these when you arrive.

» Dress Code and Packing ListThis document provides an outline for what your participants should bring with them to the project.

Applications and Forms

» Background Check Authorization Form (Online Version) or (Paper Version)This form is for all adults and participants 18 and older that are NOT serving in a leadership role. It gives Mission Serve permission to do a background check. 

» Volunteer Application (Online Version) or  (Paper Version) – This form is for adults who want to serve in one of our leadership roles. The online version of the application will automatically be sent to the coordinating team. For: Construction Captains, Squad Leaders, Supervisors and Runners. After applying to volunteer in a leadership position, your application will go through an approval process. You will be notified if you have been selected to lead.
